Core Wellness Centre offers self-pay appointments and may be able to submit eligible claims directly to participating insurance providers and programs.

Before care begins, the clinic or practitioner will explain the applicable fees. Insurance coverage and payment decisions remain with the insurer or program.

Paying for Your Appointment

Patients may pay for their appointments directly and receive an itemized receipt.

Contact the clinic before your appointment to confirm:

  • The current fee for your appointment

  • The accepted payment methods

  • Whether direct billing may be available

  • Any required referral or prescription

  • The cancellation and missed-appointment policy

  • Fees for reports, forms, products or other services

Fees will be explained before the related service or product is provided.

Extended Health Benefits

Your employer or personal health plan may provide coverage for services such as:

  • Chiropractic care

  • Physiotherapy

  • Registered massage therapy

  • Custom orthotics

  • Compression socks or stockings

Coverage depends on your specific plan.

Your plan may include:

  • An annual coverage limit

  • A limit per appointment

  • A deductible

  • A co-payment

  • A required referral or prescription

  • Restrictions on particular services or products

  • Requirements concerning the practitioner’s credentials

Direct Billing May Be Available

Core Wellness Centre may be able to submit eligible claims directly to participating insurers.

To submit a claim, the clinic may need:

  • Your insurer’s name

  • Your policy or plan number

  • Your member or certificate number

  • The policyholder’s information

  • A referral or prescription, when required

  • Your authorization to submit the claim information

Submitting a claim does not guarantee approval or payment.

The insurer may:

  • Pay the clinic directly

  • Pay the patient

  • Cover only part of the fee

  • Request more information

  • Decline the claim

You remain responsible for deductibles, co-payments, amounts above your coverage limit and any portion the insurer does not pay.

Can the Clinic Verify My Benefits?

The clinic may be able to view limited claim information through an insurer’s billing system.

This information may not include:

  • Your complete remaining balance

  • Other claims that have not yet been processed

  • Every plan exclusion

  • Future eligibility

  • Final approval of the claim

Only your insurance provider can confirm the terms of your plan and make the final payment decision.

When Direct Billing Is Not Available

If the clinic cannot submit your claim directly, you may pay for the appointment and receive a receipt.

You can then submit the receipt to your insurer, subject to your plan’s rules and eligibility requirements.

Previous reimbursement does not guarantee that a future claim will be approved.

WSIB Claims

WSIB may cover approved healthcare services related to an eligible workplace injury or illness.

Having a WSIB claim number does not mean that the claim or every proposed service has been approved.

Please provide the clinic with:

  • Your WSIB claim number

  • Your date of injury

  • Your employer’s information

  • Your case manager’s contact information, if available

  • Relevant referral or claim documents

WSIB determines eligibility, required authorization and payment.

Motor Vehicle Accident Claims

Eligible medical and rehabilitation services following a motor vehicle accident may be paid through automobile accident benefits.

The automobile insurer determines:

  • Whether the claim is eligible

  • Which services require approval

  • Applicable limits

  • Required forms and treatment plans

  • The amount the insurer will pay

Please bring your claim number, insurer information and adjuster’s contact details.

Core Wellness Centre may assist with eligible forms and billing. The clinic cannot guarantee that the insurer will approve a proposed service or pay the submitted claim.

Custom Orthotics and Compression Garments

Insurance requirements for orthotics, compression socks and compression stockings vary.

Before purchasing, ask your insurer:

  • Whether the product is covered

  • Whether a prescription is required

  • Which professionals may prescribe or dispense it

  • Whether a biomechanical assessment is required

  • How often the product may be replaced

  • What supporting documents must accompany the claim

A clinical recommendation does not guarantee insurance reimbursement.

Payment Arrangements

If the clinic offers a prepaid or multi-visit chiropractic payment arrangement, you will also have the option to pay for each service as it is provided.

Before agreeing to an arrangement, you should receive written information explaining:

  • The services included

  • The regular cost per service

  • The cost per service under the arrangement

  • Services or products not included

  • Your right to end the arrangement

  • How the unused portion will be calculated and refunded

Agreeing to a prepaid arrangement is not required to receive chiropractic care.

Receipts and Itemized Accounts

The clinic will provide a receipt for payments made.

You may request an itemized account showing:

  • The service provided

  • The date of service

  • The practitioner who provided the service

  • The amount charged

  • The amount paid

  • Any product or additional service included

Keep your receipts and insurance statements for your records.

Contact the clinic before booking if you have questions about fees or payment.  The Core Wellness Centre team can explain its current fees and whether any payment arrangements are available. The availability of a particular arrangement cannot be guaranteed.

Chiropractic services at Core Wellness Centre are provided by Dr. Kris Dorken, DC.

Dr. Kris is registered as a chiropractor in Ontario and is a member of the College of Chiropractors of Ontario.

Frequently Asked Questions

Do I need insurance to book an appointment?

No. Self-pay appointments are available. Contact the clinic for current fees and accepted payment methods.

Does direct billing mean my appointment is fully covered?

No. Your plan may include limits, deductibles, co-payments or exclusions. You are responsible for any portion the insurer does not pay.

Can the clinic guarantee my insurance claim?

No. Your insurer or program makes the final decision about eligibility and payment.

What happens if my claim is declined?

You will be responsible for the unpaid balance. You can contact your insurer to ask why the claim was declined and whether more information is required.

Do I need a referral?

A referral may not be required to book an appointment, but your insurance plan may require one for reimbursement.

Can I pay for each chiropractic visit separately?

Yes. Patients must have the option to pay for each chiropractic service as it is provided, even when another financial arrangement is offered.
